
Person-centred new care models - WP9
The main goal of this work package is to identify unmet needs in best practices of emerging European new care models, by reviewing and analysing best practices in the field of dementia and other neurological disorders, compare them with existing guidelines and optimize person-centred new care models.
The specific objectives of WP9 are:
- To optimize new models to support economic and social sustainability.
- To identify how healthcare professionals can improve the quality of treatment for patients with dementia and non-communicable diseases to maintain or increase patients' autonomy levels and decrease family burnout .
- To include families, caregivers and stakeholders in all stages of the project giving them an important role.
- To combine the aging-in-place model (as long as possible in own home or in other residentials on own conditions) with new technologies and people-cantered care models.
