About the pilot
Patients will be screened using appropriate questionnaires. If a diagnosis is made, the patient will receive information on how to prevent the progression of the disease, with a strong emphasis on lifestyle changes. If it is determined that the patient is at risk of developing a disease leading to dementia, he or she will receive training to prevent the development of this risk. These activities will be undertaken using electronic and paper tools. At specific time points, patients will be assessed using online tools regarding the implementation of actions to prevent the development or progression of risk factors leading to dementia.
Objectives
Primary objective
To test a new aproach - technique/tools - of reducing the risk factors of development of dementia.
Expected outcomes
The pilot is expected to improve identification of individuals at increased risk of dementia and increase awareness of modifiable risk factors. It aims to enhance understanding of preventive information, support motivation for lifestyle change, and facilitate appropriate follow-up in primary care. The evaluation focuses on participants’ experiences of information and follow-up, as well as feasibility and usability in clinical practice
