Implementation of narrative care supported by digital tools for Person-Centered Care in dementia

Lead entity: FUNDACION PARA LA FORMACION E INVESTIGACION DE LOS PROFESIONALES DE LA SALUD DE EXTREMADURA

This pilot aims to evaluate a Person Centred Care approach supported by digital narrative care tools for people living with dementia. Using the Amaia Cuida digital platform, residents, relatives and professionals will collaboratively collect and structure life-story information, including personal experiences, preferences, values and routines. This information will be integrated into individual care plans to improve personalisation of support, strengthen identity preservation and enhance relationships between residents, families and staff. The pilot includes staff training, digitalisation of life stories, implementation of personalised support plans and impact evaluation. Outcomes will focus on quality of life, care personalisation, caregiver well-being, professional practice and the feasibility of scaling the model to other care settings.

Silent Alarm Systems

Lead entity: AARHUS KOMMUNE

The Silent Alarms pilot aims to reduce noise-related stress and alarm fatigue in dementia care settings. Existing alarm systems used in Danish care homes often generate frequent audible notifications that can disturb residents and contribute to stress and cognitive overload among staff.

The pilot originally sought to identify silent alarm solutions available on the market. When no suitable solutions were identified, a market dialogue was initiated to explore the development of new solutions. Due to budget restrictions and strategic changes, the focus shifted towards assessing existing monitoring and watch-based technologies that may meet many of the identified needs.

The pilot will be implemented at Skovvang Dementia Care Home in Aarhus and involves evaluating selected technologies that can deliver preventive mobility and fall-related notifications in a more discreet and dementia-friendly manner. Activities include needs assessment, regulatory review, supplier engagement, establishment of data processing agreements, and testing of selected solutions in real care settings.

Testing a digital solution for in-house and ambulatory monitoring safety

Lead entity: BYLGARSKO DRUZHESTVO PO DEMENZII

The Bulgarian pilot aims to utilize a digital solution for in-house and ambulatory monitoring safety. The pilot will be conducted through wearable devices - pairs of smart watches with sensors and a specific set of monitoring features, connected to smartphones. The goal is to capture physio-behavioral and environmental factors among patients with dementia.

Patient data will be collected through these digital solutions and then stored in a GDPR-compliant way, securely encrypted and analyzed. As software infrastructure facilitating this monitoring process and serving as a repository for patient data and analysis capabilities, the Bulgarian pilot will join the proprietary platform of the Medical University of Warsaw (MUW), who also have a similar pilot action within WP9.

Optimizing Post-Diagnostic Care in Dementia and Neurodegenerative Disorders via Telecare

Lead entity: LIETUVOS SVEIKATOS MOKSLU UNIVERSITETAS

Eligible patients from LSMU Kaunas Clinics' Neurology Department sign informed consent and attend an initial visit (30 min) for an anonymous questionnaire on symptoms, lifestyle, medication, and attitudes toward remote care. Weeks 1 and 12 include 24-hour ambulatory blood pressure monitoring. Over 3 months, participants use the Viallies platform (www.Viallies.com) daily via a personal account for questionnaires and remote consultations, plus validated devices: blood pressure monitor (2x daily), smartwatch (12+ hrs/day), smart scale (daily), and sleep monitor (nightly), collecting blood pressure, heart rate, ECG, body composition, sleep, activity, and intake data, with medication reminders. The platform supports caregivers and care-team contact. Neurologist consultations review data and treatment effectiveness, with remote follow-ups as needed. Devices are free for 3 months. Data is securely managed per regulations, accessible only to authorized researchers. Procedures are non-invasive and clinically validated.

Working title "Potential molecular biomarkers identified using omics techniques for monitoring of disease progression and quality of life worsening"

Lead entity: INSTYTUT BIOCHEMII I BIOFIZYKI POLSKIEJ AKADEMII NAUK

The Pilot aims in the increase of patients and health care professionals (HCP) literacy regarding the identification of potential molecular biomarkers for neurodegenerative diseases progression using omics technologies, their use for the diagnostics of quality of life worsening and for disease progression. The Pilot will have the form of Workshop, interactive lecture and leaflets provided to Pilot participants. Special emphasis will be given to the improvement of HCP literacy, HCP encouragement to participate in studies, enhancement of credibility of clinical trials/study, improvement of data quality and understanding disease molecular biology. In case of patients the Pilot will lead to the improvement of their literacy and encouragement of their participation in clinical trials-improved recruitment. This is expected to lead in the future to enhanced access to patients’ material and improved trust to scientists and research. In case of patients’ families the Pilot aims to help patients to participate in trials/study. All target groups will become aware about the possibility of combination of biomarkers and other data collected by digital tools in the future studies to improve monitoring of the disease progress or quality of life. The participants' knowledge/literacy will be originally tested before participation in the Pilot by short anonymised survey and the increase of the literacy will be assessed by the short exit poll.
